Section A: Who can file a Request to Review an Order
Any party to an order issued by the Landlord and Tenant Board (LTB) may file a request to review an order. As well, any other person directly affected by the outcome of a particular order may file a request to review the order.
The LTB will only consider a request to review a final Order, or an Interim Order which affects the rights of a party in a final way.
Section B: Deadline for filing a Request
A request to review an order or decision issued by the LTB on or after July 1, 2026, must be made no later than 15 days after the date the order was issued.
A request to review an order or decision issued by the LTB before July 1, 2026, must be made no later than 30 days after the date the order was issued.
If the request is made after the deadline, the requestor must also make a request to extend time. The Request to Extend Deadline is available from the LTB website at tribunalsontario.ca/ltb.
Section C: Reasons for making a Request
Before filing a Request to Review an Order, you should review the LTB’s Interpretation Guideline 8: Review of an Order, and Rule of Practice 26 (Review of Orders) to assist you in completing your request. This Guideline and Rule of Practice are available from the LTB website at tribunalsontario.ca/ltb.
If you are a party affected by an order, you can make a Request to Review an Order only in the following circumstances:
- the party was not reasonably able to participate in the proceeding for one of the following reasons:
- I did not receive adequate notice of the hearing or of the issues to be decided at the hearing.
- The LTB did something that contributed to my inability to participate in the proceeding
- I was misled by another party to the proceeding.
- I was affected by a serious emergency, such as,
- a serious illness, a serious injury or hospitalization, or
- the serious illness, serious injury, hospitalization or death of an immediate family member.
- I was incarcerated or incapable.
- There was natural disaster or other similar circumstance.
- the order is the consequence of one of the following types of serious errors:
- The LTB acted outside of its jurisdiction or committed a material breach of procedural fairness.
- The LTB made an error of law, fact or mixed fact and law.
- The LTB ordered a remedy that is significantly outside of the range of usual and proportionate remedies for the type of proceeding.
- The terms of the order are unenforceable.
- the party has obtained new evidence that:
- was not before the LTB during the proceeding;
- could not have been obtained before the order was made, and
- would likely have affected the result of the proceeding had it been before the LTB.
The Request to Review an Order is not an opportunity for a party to have their matter heard a second time if they are not satisfied with the LTB’s decision.
Section D: How to complete a Request
Read these instructions before completing the application form. You are responsible for ensuring that your application is correct and complete. Follow the instructions carefully when you complete the application form.
It is important to follow the instructions below:
- Print clearly or type into each section.
- Do not fill in sections that do not apply to you.
- If the instructions tell you to check a box or a circle (for example, circles marked “Yes” or “No”), mark the box or circle selected clearly.
Language preference
The LTB offers services in both English and French. If you, your representative, or one of your witnesses would like to receive French Language Services, please select French.
Request for accommodation

Accommodations are arrangements to allow everyone, regardless of their abilities, to participate fully in the LTB's process.

Part 1: General Information
Requestor's Information
Check the appropriate box to indicate whether you are a landlord, tenant, co-op, co-op member or other party.
Fill in your name and address. If the requester is a company, fill in the name of the company under "Company Name". Include the telephone number and e-mail address, if you have them.
Unit, Building or Complex Covered by the Request
Enter the full address of the rental unit, including the unit, apartment, or suite number and the postal code.
If the street name includes a direction (ex: Mayfield Road North), you should include it. However, if there is no street direction, you do not need to add one.
Example:
If the address is: #208 at 1120 Mayfield Road North, London, N6J 2M1, this is how you should complete Part 1 of the application:

Other Parties to the Application
Check the appropriate box to indicate if the other party to the request is the landlord, tenant, co-op, co-op member or other party.
Fill in the other party’s name and address. If the other party is a company, fill in the name of the company under “First Name”. Include both daytime and evening telephone numbers and a fax number and e-mail address, if you have them.
If there is more than one other party, provide the names, addresses and telephone numbers of the additional other parties on the Schedule of Parties form which is available from the LTB's website at tribunalsontario.ca/ltb.
Part 2: Reasons for your request
Check the appropriate box or boxes to indicate the reason(s) you are filing the review request.
In the space provided, explain in detail why you think the review request should be granted. As well, indicate how you think the order should be changed if your request for review is successful. If you need more space you can attach up to 10 double-spaced pages. You can also include evidence or caselaw that you think will help support your review request. If your review request does not comply with the requirements the LTB may refuse to accept it.
Important: A party may only file one request to review an order so it is important to include all your reasons. You must convince the LTB that:
- you were not reasonably able to participate in the proceeding for one of the reasons set out above;
- the order is the consequence of one of the types of serious errors set out above; or
- you have new evidence that you could not have obtained before the decision was made, and would likely have affected the result of the proceeding had it been before the LTB.
If the LTB is not convinced, the Request to Review an Order may be dismissed without further consideration.
On the form, check the appropriate box to indicate if you are asking the LTB to stay (put on hold) the order you want reviewed, or lift (remove) a stay.
- If you are requesting that the LTB stay (put on hold) the order you want to review then, in the space provided on the form, explain why the order should be stayed. An order that is stayed cannot be enforced.
- If the order you want reviewed has been appealed to the Divisional Court, the order is automatically stayed and the LTB cannot consider your Request to Review an Order unless it first decides to lift the stay of the appeal. In the space provided on the form, indicate if an appeal has been filed with the Divisional Court and explain why the LTB should lift (remove) the stay resulting from the appeal.
Part 3: Signature
If you are the requester, check the box marked “Requester”. Then, sign the review request and fill in the date.
If you are the representative check the box marked “Representative”. Then, sign the review request and fill in the date.
Payment Method
Check the appropriate box to show whether you are paying by money order, certified cheque, or credit card (Visa or Mastercard).
If you are filing by mail or courier, you can pay by credit card by completing the Credit Card Payment Form and submitting it with your application. You cannot pay by cash or debit card if you are filing your application by mail or courier.
Your application will not be accepted if you do not pay the application fee at the time you file the application. If you owe money to the LTB as a result of failing to pay a fee or any fine or costs set out in an order, your application may be refused or discontinued.
You may request a fee waiver if you meet the financial eligibility requirements set out by the LTB. You will need to complete the Fee Waiver Form which is available from the LTB website at tribunalsontario.ca/ltb. Section E: What to include when you file a Request
To file this request, you must include the following:
- The completed Request to Review an Order form,
- The application fees. For a list of the LTB's fees visit the Filing and fees webpage.
Your request to review an order will be refused if you do not pay the application fee.
Section F: How to file a Request
You can file your application in one of the following ways:
- File by Email, Pay Online
You can pay online using a debit or credit card. If you pay online, email your receipt and application to LTBpayments@ontario.ca.
- By Mail or courier
Mail or courier your L1 application to the nearest LTB office. To find a list of LTB office locations visit the LTB website at tribunalsontario.ca/ltb. You can also call the LTB at 416-645-8080 or 1-888-332-3234 (toll-free).
If you mail or courier your application, you can pay the application fee by certified cheque, money order, Visa or MasterCard. Certified cheques and money orders must be made payable to the Minister of Finance. If you are filing by mail or courier and paying by Visa or Mastercard, you must complete the Credit Card Payment Form and submit it with your application.
- ServiceOntario Centre
Some ServiceOntario Centres accept all LTB applications in-person. If you are filing at a ServiceOntario Centre, you must complete the PDF application and pay the filing fee by Credit or debit card (backed by Visa or Mastercard) using the Credit Card Payment Form, by including a Certified Cheque or Money Order (payable to the Minister of Finance), or by cash.

Due to Payment Card Industry (PCI) security compliance requirements, the LTB cannot accept credit card payment via fax. Applications with credit card information will be automatically deleted and not processed. Please contact the LTB at 1-888- 332-3234 for information on how to submit payment information.
