Due to a labour disruption at Canada Post, mail services have been impacted at some of our tribunals.
Parties are encouraged to use email when submitting documents to the tribunal. Documents may be sent by courier if email is not an option. If you, or a party, are waiting to receive documents from a tribunal, email or a courier may be used.

Forms
To appeal or make an application to the Fire Safety Commission (FSC), you must complete the appropriate forms and provide the necessary details in writing. You should provide reasons for your appeal or application and the result or action you are seeking.
To avoid delays in processing your appeal, you should provide:
- your full name, address, email address and telephone number
- if you have chosen to have legal counsel or an agent to represent you, their full name, address, email address, telephone and fax numbers
You can download forms to your computer by right clicking on the PDF icon and selecting “Save link as” and then opening the PDF form on your computer in a PDF reader. Fill out the form and submit by email, mail or courier. Do not open the forms directly in your internet browser (e.g., Chrome, Safari). Some browsers will not allow you to complete and save the form.
You are responsible for making sure your form is complete and correct and that it includes the required details and information. If you have questions about a form, you can contact the FSC at 416-326-1356 or toll-free at 1-888-444-0240.

How to file
You can submit forms to the FSC by email, mail or courier.

If you are unable to submit your completed forms by email, you can submit them by mail or courier to:

If you submit by mail or courier, you will experience processing delays.
Some forms and documents may temporarily contain old addresses. Disregard the old addresses and send all forms and documents to the address listed above.
Fees
There is no fee to file an appeal or application with the FSC.
