The SBT has updated the Appeal (Form 1) and Application for Reconsideration (Form 2) to include the Social Assistance Intake Unit as a Respondent for appeals related to applications for both Ontario Disability Support Program (ODSP) and Ontario Works (OW).
In Part 2 of the Application for Reconsideration (Form 2), the form has been updated to include Representative Information.
